Q. How do I apply?

A. You have a choice. Employees are given two opportunities to enroll with no medical underwriting. The first opportunity is within the first 30 days of employment, and the second opportunity is during the first annual enrollment held after your date of hire. If you decline coverage during these initial periods, you can apply at any time in the future by providing satisfactory medical evidence of insurability.

Q. When do benefit payments begin?

A. If your claim is approved, benefits would begin after the waiting period. Your plan’s waiting period is 180 days after your disability begins.

Q. What happens if I leave the United States while on claim?

A. You will not be required to remain in the United States to be eligible to receive a benefit for the full benefit period - if a covered disability occurs while you are employed by Cognizant and all related entities.

Q. How much do my options cost?

A. Premiums include a 25% discount and vary based on your age and amount of benefit.
