Q. How do I enroll?
A. Click the login button to continue to the enrollment site. Log in to view your options, apply or decline.
Q. When do benefit payments begin?
A. If your claim is approved, benefits would begin after the waiting period. Your plan’s waiting period is 180 days after your disability begins.
Q. What happens if I leave the United States while on claim?
A. You will not be required to remain in the United States to be eligible to receive a benefit for the full benefit period - if a covered disability occurs while you are employed by Cognizant and all related entities.
Q. How much do my options cost?
A. Premiums include a 25% discount and vary based on your age and amount of benefit. Log in to view your options and costs.
How to Request a New Hire Packet
Q. How do I request a new hire enrollment packet?
A. Send an email to customerservice@hfcb.com to request your new hire enrollment packet.
Q. After requesting, when should I expect to receive my enrollment packet?
A. You should expect to receive your personalized enrollment packet at your home address within 15 business days.
